MSHA 5000-23 Certificate of Training
Use this form to document training completion for Part 46 or for Part 48. For Part 46, you have the option of creating your own compliant Record of Training document or using this 5000-23. For Part 48, you’re required to use the 5000-23 for to document training completion.

MSHA 5000-46 Request an MSHA Individual Identification Number (MIIN)
Allows MSHA personnel mine operators, miners, independent contractors, and other MSHA qualified and certified individuals to apply for an MIIN.
MSHA 7000-51 Mine ID Request
Allows mine operators to apply for and be issued an MSHA ID. This must be done before any mining operations begin.
